Model of care

Primary health care

Since 1996 we have had a service agreement with the ACT Government, our primary funder. This specifies that we must provide primary health care services to government standards.

Our primary health care model promotes physical and emotional wellbeing and incorporates services that are:

C-Frame covers how we deliver primary health care. It is a strengths-based, solution-focused approach for working with families and with each other.

The name C-Frame encapsulates the three action c-words embedded in the framework—connect, collaborate and change.

By using the C-Frame, we work together to:

  • ensure your values and goals are clarified
  • identify your needs, strengths, skills, competencies and goals
  • work with you to make the right changes and meet your goals
  • motivate you to actively participate in your care
  • ensure we engage in regular, scheduled and ongoing reflective practice.

Helping families thrive

We wish to acknowledge the Ngunnawal people as the traditional owners and custodians of the land on which CMS and its services at QEII are located, and their Elders past and present.
